How To Qualify

How to Qualify 

The Lead Hazard Control program aims to provide a public health benefit through housing to low-income families and to aid in that goal, there are some differing qualification requirements for rental properties, and owner-occupied properties. Namely, that owner-occupied properties must have a child under 6 in the home, or an expecting parent. This is because rental properties are more likely to have a child move into the home in the near-future. Additionally, because this program directs public health benefit to low-income households, the occupant is always income-verified. This could be the owner, if it is an owner-occupied property, or the tenant in a rental property. Qualifying a Rental Property:

  • Be located in Burlington or Winooski
  • Have been constructed prior to 1978
  • Have at least one (1) bedroom
  • Total household income cannot exceed 80% of the area median income. Income will be verified.

  • Must have a child under 6, or expectant parent, living at the residence or visiting frequently.

 

Apply 

In order to apply, submit the following documentation:

  • Property Application: This application asks questions about the property and is filled out by the owner. Fill out the Property Application through OpenGov or download the PDF version.
  • Household Application: This application asks about the household demographic and income information. This should be filled out by the occupant, which could be a tenant in a rental property or the owner in an owner-occupied property. Landlords are responsible for collecting the application from their tenants. BLP staff will attempt to collect additional income documentation. Fill out the Household Application online through OpenGov or download the PDF version.
  • Property Insurance Policy: The cover page of your insurance policy, showing that you have obtained an insurance policy for the property.

Applications and documentation can be submitted to the Burlington Lead Program through OpenGov (linked above) or a digital .pdf can be sent through email to blp@burlingtonvt.gov. We also accept physical mail or drop-off to:

Income Verification Forms                                                                                 

Please send all PDF forms to the Burlington Lead Program at blp@burlingtonvt.gov or send physical copies to our address at 149 Church St. Room 32 Burlington, VT 05401. Webforms can be submitted online; if a webform is available for a particular form, you may fill that form out online instead of sending in the PDF version. 

If you are employed, please provide a signed Verification of Employment Form. Please fill out the top half of the form and send it to us; we will then send the form to your employer to be completed.

If you receive gift income/bills paid on your behalf, please provide a signed Gift Income Affidavit. This includes situations where family members are paying rent or phone bills.

If you are self-employed, please provide a copy of your most recent tax return as well as a complete Self-Employment Affidavit Form.

If you receive any public assistance income (unemployment benefits, SS, SSI, etc.), please provide either a letter from the benefit provider verifying your unemployment benefits (preferred) or a screenshot of your benefits account page showing the total annual amount received (an example of both the Vermont Department of Labor letter & portal page screenshot is below).

If you receive a Section 8 Voucher, please return the completed Household Application form with the housing agency provider listed on the back.

If you have more than $5,000 in assets, please provide us with a statement describing the assets and specifying their value.
